Forms
Off by default
Collect applications, reports and sign-ups through Discord pop-up forms or on the web. Staff review each submission and can hand out roles when they accept.
Set it up
Open the dashboard and pick your server. Sign in with Discord if you haven’t.
Find Forms in the sidebar (or press ⌘K and type its name) and turn it on.
Go through the settings below. Simple mode shows the essentials; switch to Advanced for the rest.
Try it in Discord:
/form. Staff can restrict or turn off each command in Commands.
Tips
- Use a web form when you need more than five questions.

What you’ll find on the Forms page of the dashboard. Settings marked Advanced only show in Advanced mode.

- RolesMembers with these roles can accept or deny submissions of every form. Moderators always can.
- Choices in DiscordAdvanced
- ChoiceRadio buttons and checkboxes are easier to use, but Discord allows them only up to 10 options; longer lists always use a dropdown.
